Geriatric Care Management Can Improve Elder Care

“Geriatric care managers have extensive knowledge of elder issues and the resources in the community. Broadly speaking, a Geriatric Care Manager’s (GCM) role is to serve as an advocate for the senior. GCMs may have a degree in a field such as nursing, social work, gerontology or psychology.

The GCM is responsible for identifying the needs of the individual, developing a care plan and helping the senior access resources outlined in the care plan. The GCM also monitors care as time goes on.

GCMs are particularly helpful for seniors who have complex needs, because they are able to assess and address the diverse aspects of an individual’s life. A GCM may also be called in to address a relatively limited issue, such as finding a caregiver.

A GCM’s duties often include coordinating and explaining medical care and medications, home health aid, access to transportation, arranging for legal services, ensuring a safe living environment, identifying appropriate placements, physical therapy and advising on the financial aspects of care. A GCM may also be involved in resolving disputes between family members regarding long-term care.

Geriatric care management is often utilized by families who do not live in the same city as their loved one and cannot directly supervise care. However, even families who live nearby and who are actively involved in care can benefit from geriatric care management services.
